All students attending an Education program at the Martime Museum of San Diego need to have a signed Risk and Liability Form. Overnight programs require a signed medical form as well. These forms need to be turned in to the field trip coordinator/teacher ahead of time.
Overnight Programs only:
All students attending an overnight Education Program at the Maritime Museum of San Diego also need a signed Medical Form in addition to the above Risk and Liability Form.
