- Crew List
Use this form to sort your students into different crews.
Forms for ALL participants:
- Risk and Liability Form
Every participant — student, adult, or teacher — must have one of these forms filled out. - Student Medical Form
A copy of this form must be filled out for every student participating. - Adult Medical Form
Every adult — chaperone or teacher — must fill out one of these forms. - If applicable: Administration of Medication, adult or child, as needed
Any participant — student, chaperone, or teacher — who will or might take medication during the program must have this form filled out. This includes both prescription and non-prescription drugs.
